Respected Sir/Madam,

I am ______________ (Name) from _______________ (Name of the company).

I want to access the sales tax application on the website. Our company has been regularly submitting the tax to the government. Since the log in done yearly, now I, on behalf of my company and sales department, have to submit the tax of ___________ (Amount) with interest rate ___________ (Percentage rate).

But there seems to be problem while logging in. even after repeated trails, I am not able to log in because of wrong password.

It is a kind request to send the reset password on ______________ (Physically / Email ID) at the earliest. As the time is approaching and as responsible citizens we have to finish our job before time. It is a humble request to send the options/link at the earliest.
